- Refill Requests will be processed by, either;

- a patient completing the following Refill Request Form
- or an electronic Refill Request by the patient’s pharmacy
- or fax Refill Request from the patient’s pharmacy to our fax #855-295-2686

- There’s a $25.00 charge to process a refill request for less than 5 Business Days
- Refills will be until the patient’s next appointment. The patient needs to make sure to have an appointment with the provider when submitting a refill request
- Certain refills request may not be processed without an appointment with the Provider
- Provider retains the right to deny the Refill Request pending the toxicology results or to request to have the appointment scheduled before the Refill may be processed
