• Refills request will be processed after receipt of an electronic request or fax from the patient’s pharmacy at our fax #855-295-2686.
  • Refills request should be made at least 1 week prior to running out of medications to make sure patients are not without medications.
  • No-Charge when a refills request to be processed in 5 business days or more.
  • There’s a $25.00 charge when a refills request to be processed in 4 business days or less.
  • Certain refills request may not be processed without an appointment with the provider.
  • Clinicians will make a decision if UDS is needed for the refills or not.
